36 years old banker, previously fit and healthy. During his vacation in Tanzania over Christmas, he experienced a sense of fullness and hearing impairment on the left side on December 22, 2023. He also developed mild vertigo and left sided tinnitus. Upon returning to the UK, he underwent an audiology evaluation and was assessed by three experienced ENT specialists, who confirmed the diagnosis of Idiopathic Sensorineural Hearing Loss. ​MRI head showed no abnormality.

Chronology: 

02/01/2024, Day 11 of hearing loss:

The ENT specialist prescribed Prednisolone at 60mg, following the first audiology test below.

09/01/2024, Day 18 of hearing loss:

 7 days course of high dose Prednisolone was completed. The tinnitus and vertigo improved, though the patient still felt lightheaded. 

Repeat Audiology tests showed some mild improvement. However, there was still a moderate sensorineural hearing loss of 55dB on the left side at lower speech frequencies. 

Three intratympanic injections were administered on 1st 10/01/2024, 2nd 12/01/2024, then 3rd 15/01/2024. 

11.01.2024, Day 21 of hearing loss:

The patient attended NUMA. At this stage, he had finished a week of high-dose Prednisolone. Understandably, he was very anxious about the lack of response and worried about permanent hearing loss. 

From January 11th to January 22nd, 2024, he underwent 10 HBOT sessions lasting for 90 minutes each, with air breaks, except on Sundays.

He tolerated well with no adverse effects. 19.01.2024, hearing was found to have been restored, as indicated by the audiology report below.

Insights from Dr Nur: 

  1. During my first meeting with the patient, who arrived on day 21 of experiencing hearing loss, I couldn’t offer much encouragement regarding HBOT’s potential success. Recent research underlines the importance of timely treatment, advising hyperbaric oxygen therapy within 72 hours of hearing loss onset. But some studies still encourage the use of HBOT as a salvage therapy later stages when all failed, and this patient still responded despite presenting late
  2. HBOT is not a replacement for standard ENT treatments. We ensure that all our patients have an ENT specialist on the board.
  3. HBOT can be safely administered along with intratympanic membrane injections and oral steroids. 
  4. Timely diagnosis and management, the effective use of HBOT in combination with standard ENT therapies are crucial to improve the possibility of regaining hearing.
